What is the history of Commonwealth Health Foundation?
The Commonwealth Health Foundation was formed in December 2000 for the purpose of fostering, supporting and initiating activities for the advancement of the healthcare objectives of Commonwealth Health Corporation. The establishment of the Foundation allows CHC to promote volunteerism and philanthropy through community participation and partnerships. By raising funds to provide programs, services and facilities in our community, the Foundation will enable CHC to be an innovative leader for community health improvement and a lasting legacy for health in our region.

What is the mission?
To support the not-for-profit healthcare objectives of Commonwealth Health Corporation.

Is the Foundation a not-for-profit organization?
Yes, the Foundation is a 501 (c)(3) not-for-profit organization established as the fundraising arm of Commonwealth Health Corporation. It is governed by a volunteer Board of Directors.

How is the Foundation structured?
Development and community programs are carried out through a six committee structure of community volunteer leaders: Major Gifts, Special Prospects, Annual Support, Community Relations, Business Relations and Planned Gifts. These volunteer leaders are supported by an Executive Director and Development Director.

What is the current project focus?
Commonwealth Health Foundation’s philanthropic program is currently focused on attracting national, regional and local support for the Commonwealth Health Free Clinic.

The Commonwealth Health Free Clinic provides basic medical and dental services to individuals who are in the work force but do not have insurance or any form of social assistance and do not have the means to pay for their healthcare. These individuals are making an effort to help themselves; however, many times they must choose between medical care and food for their table.

Can I direct my gift to a designated area of healthcare other than the current project?
Yes, you may direct your gift to any area that you so desire. If you do not see your particular area of interest listed on the website, a section titled "other" is open for you to direct your gift as you wish.

Does my entire gift go towards the current project or area I designate?
Yes! Commonwealth Health Corporation covers any expenses incurred by the Foundation. Therefore, 100 percent of your gift goes directly toward the project or area designated.  No funds are used for administrative overhead.

How do I make a gift to the Foundation?
This website contains information on how to make a gift.  If you have additional questions, please call the Foundation at (270) 796-5543 or email us at info@chfbg.org.

Will my gift to the Foundation be tax deductible?
Contributions are tax deductible as allowed by current law. We recommend that you check with your personal tax advisor as to the deductibility of your gift.

Can I contribute in honor or memory of someone? If so, how is my gift acknowledged?
Yes, we encourage commemorative gifts. After we receive your gift, we send an acknowledgement of the gift to the donor, as well as to the family or person designated. The gift amount is always kept confidential.

Can I make an anonymous gift?
Yes! The Foundation always honors donor's wishes. If the donor wishes to remain anonymous, the name of that donor will not be used on any donor listings.
