You can print out the Gift Form* and mail it
with your gift or you can make a
secure donation online.
Donations can be made in memory, in honor or in celebration of a birth,
anniversary, or birthday. An acknowledgement of the gift is sent to the family
or person designated. The gift amount is always kept confidential.
While the current project focus is to support the
Commonwealth Health Free Clinic,
you may direct your gift to any area that you so desire. Commonwealth Health
Corporation covers any expenses incurred by the Foundation. Therefore, 100% of
your gift goes directly toward the project are designated. No funds are used
for administrative overhead. Call the Foundation at (270) 796-5543 for more

The General Joseph Warren Society is a distinguished group of benefactors who
support the advancement of healthcare services in Southcentral Kentucky.
Established in the spring of 2001and founded by the
Special Prospects Committee, the Warren Society is composed of individuals
who are recognized as healthcare benefactors of the highest order. They
recognize and believe in the importance of high quality healthcare services and
how those services impact the quality of life in our communities.

Business Honor Roll
The Business Honor Roll of the Commonwealth Health Foundation was established
to recognize businesses that support the advancement of healthcare in
Southcentral Kentucky. It was founded by the
Business Relations Committee. Funds raised through the Business Honor Roll will provide
financial assistance to support the mission of the Commonwealth Health
Foundation. The Foundation was established to support the not-for-profit
healthcare objectives of Commonwealth Health Corporation. Business Honor Roll
members will help promote the continued excellence in technology and services
provided by Commonwealth Health Corporation.

The Heritage Circle of the Commonwealth Health Foundation was established to
recognize and honor those individuals who support the advancement of healthcare
in Southcentral Kentucky. These individuals have demonstrated their
philanthropic support through planned giving.

Endowment Funds
In June 2001, the Foundation announced the establishment of the Free Clinic
Endowment Fund and an Undesignated Endowment Fund. Endowments are gifts of which
only the interest income from the principle is spent. These funds will serve as
a perpetual income source to support the services and programs of the Free
Clinic and other designated areas.
There are three types of endowment giving. The General Endowment Fund was
established for all gifts that are intended for endowment but non-directed by
the donors. A minimum gift of $25,000 is required to direct an endowed gift to a
certain area of interest. A minimum gift of $50,000 is required for the donor to
name the endowment.
A donor who makes a gift at the $25,000 or $50,000 level would not only be
recognized as an Endowment contributor, but also as a
Warren Society General’s Circle member at the 2
Star or 3 Star General level.
The following individuals have made very generous gifts to this fund:
- $50,000 gift given by an anonymous donor
- $100,000 gift to establish The Desmarais Family
Endowment given by John and Chris Desmarais
- $100,000 gift to establish The Scott Family
Endowment given by Rita and Jim Scott
- $100,000 gift to establish The Vitale Family
Endowment given by Suzanne and Don Vitale
- $100,000 gift to establish The English, Lucas,
Priest and Owsley Endowment given by English, Lucas, Priest and Owsley
- $100,000 gift to establish The Kate B. Layne
Endowment given by Bud and Jana Layne.
- $50,000 gift to establish the Carrel K. Sumner Family Memorial Scholarship
given by Clara Sumner. Commonwealth Health Corporation matched the endowment
gift with an additional $50,000 contribution.
- $6,500,013 in gifts to the undesignated endowment fund to support the
Free Clinic and other CHC nonprofit entities.
- $100,000 gift to establish The Dr. Patricia J.
Mercer Endowment given by Dr. Patricia J. Mercer.
- $100,000 gift to establish The Dr. G. Y. Graves Memorial given by the family of Dr. G.Y. Graves.
